Medication Management & Medication Therapy Management

At EQC Home Care, we have pharmacists on staff who can be your advocate and provide you Medication Therapy Management (MTM). Unlike the traditional pharmacist role of providing narrow consultations based on dispensing medications, our MTM program allows our Pharmacists to provide a full evaluation to address current and potential medical related issues. A health advocate works with you one-on-one to understand your current health issues and treatment program. They help speak on your behalf to achieve the best results for improved wellness.

Our pharmacists develop a relationship with you. We review your medical records and take note of your current health. We deliver face-to-face appointments or video conference calls to speak with you and understand your concerns. We also observe you and watch for signs of potential issues. We’re here for you to help you follow your doctor’s orders and be an advocate to help you achieve improved wellness.

Our Medication Services Include:

Medication Therapy Management – Our pharmacist works directly with you to manage medication related health issues or potential issues.

Disease Management – Our pharmacist will collaborate with your doctor to provide a medication therapy review as one aspect of managing diseases or other ailments. 

In-Home Administration – Depending on your needs, a registered nurse can come to your home and administer medications directly.

Patient Education – Our pharmacist will educate you about the medications that you’re taking and create a plan to help prevent dosing mistakes. Our registered nurses provide this service as well.

Reminders and Monitoring – For more independent clients, we offer medication support with timely reminders and dosage monitoring.
