How this website handles your information

What the callback request and job application forms collect, where the information goes, who can see it and how long it’s kept.

This page explains how the EQC website handles the information you send through its two forms: the callback request and the job application. How EQC handles its clients’ health information is described in the Notice of Privacy Practices.

What each form asks for, and why

Request a call

  • What it asks for: your name, whether you’d like a phone call or an email, your phone number and email address (you must give the one you prefer; the other is optional), and a checkbox confirming EQC may contact you about the request.
  • Why: so EQC’s team can get back to you about your question.

Job application

  • What it asks for: the position, your name, email address, phone number and preferred contact method, your answer about the role’s license or certification, when you could start, a checkbox confirming EQC may contact you about the application, and an optional experience summary of up to 1,000 characters. It doesn’t accept files, links or a résumé.
  • Why: so EQC’s hiring team can review your interest in the position and contact you about it.

Both forms also include a hidden field that people leave empty, which helps block automated spam, and a random ID your browser creates so the same request isn’t sent twice.

Where your information goes

When you send a form, it travels over an encrypted (HTTPS) connection to the website’s server, which runs on Amazon Web Services (AWS). If the server accepts it, it sends one plain-text email through Amazon Simple Email Service to a single EQC office inbox, which uses Google Workspace email. The website can’t send your information to any other address, and it doesn’t send you a confirmation email.

The form says your request or application was sent only after the email service accepts it. If the service doesn’t accept it, the form tells you it couldn’t confirm or send it and suggests calling. In rare cases, a request the form couldn’t confirm may still reach the office.

Who can see it

The email goes to one EQC office inbox. EQC limits access to that inbox to its human resources staff and its Chief Operating Officer. AWS sends the message and Google Workspace stores it, and both handle it under their own terms and privacy policies, which this page doesn’t describe.

How long it’s kept

  • The email in the office inbox: EQC keeps these emails for seven years.
  • The website doesn’t save what you enter in a database.
  • Spam and abuse limits: to limit spam and repeated requests, the server keeps one-way (keyed hash) codes made from your network address, each with a count of requests. A code and its count expire after the rate-limit window, which is one hour unless EQC changes the setting, or, for the daily count, at the end of the day (UTC). The server never saves the network address itself. It also keeps the random request ID your browser created, with its reference number and whether the email was sent, for one day. A daily count of emails sent, which contains nothing about you, is kept for two days. Expired records are no longer used and are usually deleted automatically within a few days.
  • Server logs record that a form was used, its reference number, the outcome and timings, for 30 days. They don’t contain what you typed, your name, contact details or network address. EQC has not turned on access logs for the website’s content delivery or its form service.

What not to send

Please don’t send health, diagnosis, medication, medical-record, insurance, identification or financial information through this website, including a Social Security number or date of birth. If you’re asking about care for a child, don’t enter the child’s name or health details. The forms are not for emergencies and don’t provide medical advice. For an emergency, call 911.

Cookies, analytics and other services

This website doesn’t set cookies, and it has no analytics, advertising pixels, chat tools or third-party scripts. Its fonts and images come from the website itself. The Reviews page links to EQC’s profile on Google Maps; if you open it, Google’s own privacy terms apply.

The website saves one small item in your browser’s session storage, and only in one situation. If the website’s own scripts fail to load or start and you haven’t begun filling in a form, the page reloads itself once, and the item stops it from reloading again. The item holds no information about you. Your browser deletes it when you close the tab, and the website removes it as soon as its scripts load normally.
