Can I send my medication list through the contact form?

Please do not enter medication details or medical records in the general form. Contact EQC first and ask about an approved way to share clinical information.

Are appointments in person or by video?

Medication review appointments may be in person or by video. Our team can confirm the current options and availability for your situation.

Can this website tell me to change a medication?

No. Discuss medication changes with your prescribing clinician or pharmacist. The information here explains a service, not an individual treatment plan.
