About this role

Provide physician-ordered nursing support for children and adults whose health condition is stable and predictable, involving families in care at home.

This role is part of our nursing services for people at home.

What you’ll do

• Assess health needs, stability and progress; develop and revise individualized nursing plans.
• Carry out ordered treatments, medications and diagnostic procedures within the nursing role.
• Maintain records, physician orders and clinical notes; report changes and care outcomes.
• Teach clients and families about treatment, prevention and health maintenance; provide emotional support.
• Maintain safe, hygienic care spaces and equipment; coordinate and supervise care staff.
• Monitor care and delegate only after RN assessment for the specific person, caregiver, setting and task.

What you’ll bring

• Current Oregon RN licensure in good standing.
• Knowledge of nursing rules and required employer training.
• Clear communication, sound judgment and ability to work independently.
• Computer proficiency, willingness to learn and respect for confidentiality.
• A valid driver’s license and work authorization.
• Ability to safely lift up to 50 pounds.
